磁共振3D-B-FFE序列对血管压迫性三叉神经痛的诊疗价值探究
目的：探讨磁共振三维平衡式快速梯度回波(3D-B-FFE)序列对于血管压迫性三叉神经痛的诊断价值。方法：选取58例经临床诊断为血管压迫性三叉神经痛患者，均进行MR 3D-B-FFE序列检查，将其影像诊断与手术结果进行对照。结果：58例中，术前MR 3D-B-FFE序列诊断三叉神经脑池段血管接触及受压54例，阳性率为93.1%，手术探查结果56例发现责任血管，3D-B-FFE序列显示与手术探查符合率达96.43%。结论：3D-B-FFE序列能较清楚显示三叉神经脑池段与责任血管的空间关系及三叉神经的受压情况，对于明确病因诊断及对微血管减压术(MVD)手术指征和方式的选择有着重要意义，是目前三叉神经痛术前了解责任血管理想的检查方法。
Abstract：
Objective:To investigate the value of MR 3D-B-FFE sequence in diagnosis of trigeminal neuralgia caused by vascular compression. Method:Fifty-eight patients with trigeminal neuralgia diagnosed by clinical were analyzed retrospectively. The 3D-B-FFE MR images were compared with the surgical results. Result:Preoperative MR 3D-B-FFE images showed 54 cases from all 58 patients of trigeminal nerve compression with the positive rate of 93.1%. In 58 patients,56 cases were found responsible vessels,3D-B-FFE sequence showed a lesion and operation in line with the rate of 96.43%. Conclusion:3D-B-FFE sequence can clearly show the spatial relationship between the trigeminal nerve cisternal segment and the responsible vessels,which is important for diagnosis and treatment. MR 3D-B-FFE sequence is an ideal preoperative examination for trigeminal neuralgia caused by vascular compression.

MRTA 在血管压迫性三叉神经痛病因诊断中的价值
目的：探讨M RT A检查方法在三叉神经痛患者中神经血管压迫表现及其在病因诊断中的价值。方法对28例三叉神经痛患者的M R表现与手术结果对照，并进行回顾性分析。结果 M R图像显示，23例患者存在血管压迫或接触关系，5例无接触关系。手术结果显示，M RT A在诊断三叉神经痛血管压迫或接触的准确率为82％。结论 M RT A能较清晰显示三叉神经痛病损神经与责任血管压迫的解剖关系，在三叉神经痛神经病因诊断中具有较高的价值，在为临床提供术前评价和指导手术方案方面具有重要的意义。
Abstract：
Objective To evaluate the MR findings and diagnostic value of neurovascular compression in patients with trigeminal neuralgia .Methods MR manifestations and operative results of 28 patients with trigeminal neuralgia were studied retrospectively .Results Of all 28 diagnosed patients’ M R imaging ,23 cases had vascular compression or contact and 5 had no vascular contact .The accuracy of MRTA diagnosis of vascular compression or contact was about 82% .Con‐clusion MRTA demonstrates the relation of compression between trigeminal nerve and the responsible vascular clearly .It showed high diagnostic value of vascular compression or contact in patients with trigeminal neuralgia .
