Background:DistalesophagealacidexposurecanbequantifiedusingtheDeMeesterscore(DS)orpercentagetimepH&4.Inadditiontotheseparameters,symptomindex(binedanalysis(CA)includingtheSIandpercentagetimepH&4providesamorecomprehensiveinterpretationofpHmonitoringresults.
Aim:ToassesswhetherusingCAcomparedwithDSismoreaccurateforinterpretationofpHmonitoring.
Method:pHstudiesfrom450patients(303meanage53;range11to88)offtherapybetweenJuly2002andOctober2004wereanalyzed.DS(normal&14.7),percentagetimepH&4(upright&6.3%,recumbent&1.2%),andtheSI(positivesymptomifZ50%)werecalculatedforeachpatient.Symptomresponsestoprotonpumpinhibitors(PPIs)wereassessedinpatientswithdiscordantDSandCA.
Results:Sixty-sixpatientswereidentifiedashavinganormalDSandanabnormalCA.Forty-one(62%)patientshadabnormalrefluxaccordingtotheCAand25(38%)hadnormalrefluxbutapositiveSI.Tenpatientswerelosttofollow-up,43(84%)improvedonandwerecurrentlytakingaPPIand8(16%)foundnobenefitfromtakingaPPI.Five(9%)patientswerenevertriedonaPPIand3werestillsymptomatic.Twenty-two(88%)patientswithapositiveSIandnormalrefluxwerefollowed-up(3neverprescribedPPI)and17(90%)improvedwithaPPI.Conclusions:InterpretingtheresultsofpHmonitoringbasedsolelyonthecompositionDSmissespatientswithanacid‘‘sensitiveesophagus,’’agroupofpatientswhobenefitfromPPItherapy.KeyWords:pH

astroesophagealrefluxdisease(GERD)isacommondisorderaffectingupto40%ofthegeneralpopula-tioninthewesternworld,whereas10%ofthepopulationhasdailyGERDsymptoms.1–5AmbulatorypHmonitor-ingisconsideredthe‘‘goldstandard’’forthediagnosisofGERDespeciallyinpatientswithGERDsymptomsandanormalendoscopy.6Upto15%patientswithGERDsymptomswillhaveanormalendoscopyandpHmonitoring.7Inthisgroupofpatientsthesymptomindex(SI)canbeusedtoseparatepatientswithanacidsensitiveesophagusfromthosewithfunctionalheartburn.8–10TheDeMeesterscore(DS)(compositescoreofpH&4forupright,recumbent,andtotal,numberofrefluxepisodesin24hours,numberofrefluxepisodes&5minutesandthelongestepisode)isusedbymanygastroenterologistsandgastrointestinalsurgeonsinthefinalinterpretationofpH-binedanalysis(CA)includingpHparametersandtheSIprovidesamorecomprehensioninterpretationofpH-metry.
Protonpumpinhibitors(PPIs),currentlythemostpotentinhibitorsofgastricacidsecretion,havebeensuccessfullyusedinthetreatmentofGERD.11,12There-fore,apositiveresponsetoPPItherapycanbeausefulindicatorforthecorrectdiagnosisofGERD.ThecompositeDSwasdesignedtobeveryspecificforGERD,raisingconcernsthatamoresensitivetest(liketheCA)wouldoverdiagnoseGERDandidentifypatientswhowouldnotbenefitfromPPItherapy.
TheaimofthisstudywastocompareanalysisofGERDpatients’resultsusingboththeDSandtheCAandtoevaluatehowpatientswithdiscordantresultsrespondtoPPItherapy.
MATERIALSANDMETHODS
TheInstitutionalReviewBoardattheMedicalUniversityofSouthCarolinaapprovedthisretrospectiveanalysisofprospectivelycollecteddata.
PatientPopulation
AllpatientswhounderwentambulatorypH-metrybetweenJuly2002andOctober2004offtherapywereincludedinthestudy.Patientswerestudiedforbothtypical(heartburnandregurgitation)andatypicalGERDsymptoms(chestpain,cough,sorethroat,hoarseness,abdominaldiscomfort,nausea,andglobus).AllpH-metrywithatleast20hoursofrecordingwereincludedinthestudy.PatientswerestudiedofftherapywithPPIs
